Job Summary

Enhance and support parent-child relationships by implementing a home-based Early Head Start program for pregnant moms and families of infants and toddlers. Provides individualized and comprehensive health and development services to women before, during, and after pregnancy. Partners with families to enhance the physical, social, emotional, and intellectual development of children (birth to 36 months).

Essential Functions

  • Each week conducts one home visit with the parents and children of each family, integrating information on child development, health, nutrition, mental health, and social services, promoting and supporting the families achievement of self-sufficiency.
  • Works with families to develop weekly individualized lesson plans, utilizing household materials in educational activities.
  • Completes 45- and 90-day health and education requirements.
  • Collaborates with other community agencies to provide individualized services for infants and toddlers with disabilities.
  • Plans, implements, and evaluates group socialization experiences in a center-based or community environment.
  • Provides prenatal and parenting information to expectant parents. Provides resources and/or referrals for each family as needed in areas such as health, nutrition, social services, and mental health.
  • Documents all services and follow-up treatment, as necessary.
  • Submits written reports for each home visit and socialization.
  • Completes all required record-keeping and internal and external reporting tasks in a timely and objective manner, in accordance with Child Plus and other applicable tracking systems, as requested.
  • Conducts developmental screenings and on-going assessment of the developmental progress of infants and toddlers, including parents in all aspects of services.
  • Complies with all safety policies, practices, and procedures in compliance with Head Start Performance, and FOCAF Standards.

{=html} <!-- --> - Collaborates with families, co-workers, community partners and the general public to further program goals, respond to questions, problem solve and establish effective working relationships. - Attends all scheduled child and family staffings, advising appropriate coordinator/manager of health, family services or education concerns.;. - Works with Head Start staff to support the successful transition of Early Head Start children into Head Start. - Complies with FOCAF policies regarding the reporting of abuse and neglect. - Remains up to date with policies and practices. - Effectively responds, communicates with and presents information to co-workers, customers, clients, vendors, and general public in a courteous and timely manner. - Maintains both electronic and hard copy filing systems commensurate with the needs of FOCAF and in compliance with state and federal laws and regulations. - Participates in reflective supervision at least once a month for 90 minutes. - Regularly involves and encourages parents to volunteer within all components of program. - Demonstrates commitment for professional development and continuous growth by actively seeking out and participating in training opportunities. - Engages with supervisor and co-workers on constructive and timely feedback, accepting feedback as a growth opportunity, and is open to continuous improvement for oneself and the organization.

Additional Functions

  • Provides information when requested about policies, procedures, and methods used in FOCAF operations.
  • Actively contribu tes suggestions during meetings for team collaboration.
  • Maintains shared work areas in an orderly manner and properly uses and store office materials.
  • Frequently transports to home visits and various other locations as needed to perform job duties.
  • Manages time efficiently, tolerates a high level of paperwork and documentation.
  • Maintains confidentiality.
  • Must have a valid drivers license and current automobile insurance if there is a need to travel locally during the course of the job duties.
  • Prioritizes and manages multiple tasks.
  • Problem solves and effectively mitigates conflict.
  • Ability to operate a personal computer on a network, using standard or customized software applications appropriate to assigned tasks.
  • Occasional out of local area travel to training related to Early Head Start, child development or curriculum training.
  • Performs other duties as assigned.

Qualifications

  • Home Visitor CDA, or an Associate or Bachelor Degree in Early Childhood Education, Special Education, Child Development, Social Work, Human Services or related field.
  • Two years experience working with parents, infants and toddlers from diverse populations preferred.
  • An understanding of adult learning practices preferred.
